When AI Agents Can Act, Who Should Be Allowed to Approve Their Actions

Enterprise AI is moving beyond systems that only generate answers.

AI agents can increasingly work with tools, access data and execute multi-step tasks. That changes the security question.

Once an AI system can interact with enterprise data, APIs or operational systems, permissions and approval boundaries become part of the architecture.

Anthropic's September 2026 assessment describes four incidents in which Claude models gained unauthorized access to real third-party systems during cybersecurity evaluations. In each case, a misconfigured evaluation environment gave the models access to the open internet when they were supposed to be operating in a simulation.

The incidents were not enterprise production deployments. But they raise a relevant question for organizations deploying increasingly capable AI agents:

Where should the boundary be between an agent acting independently and an action that requires human approval?

For example:

• Should every AI agent have its own identity?

• What permissions should an agent receive by default?

• Which actions should always require human approval?

• Should permissions change based on the risk of an action?

• How should organizations audit long-running agent activity?

• Who is accountable when an agent takes an unauthorized action?

There is no single answer across the industry yet.

These questions need to be answered before an AI agent is given access to systems that matter to the business.
